Hours: Pharmacists work rotating shifts and weekends. Holiday rotation required. Pharmacist works general med/surg, inpatient, and critical care coverage. Pharmacist participates in Interdisciplinary Rounds in a centralized/decentralized model. Cross campus coverage to Waukesha Memorial and DN Greenwald in Mukwonago. In addition to medication reconciliation responsibilities, pharmacist will need to staff as an inpatient pharmacist as well.

This clinical pharmacist specialist will work collaboratively with other healthcare professionals to ensure safe transitions of care within the acute care setting. The Medication Reconciliation Pharmacist will assist in creating positive clinical outcomes such as reduction in drug related adverse events, hospital length of stay and readmissions by assisting providers in tailoring medication regimens based on the best available evidence in treatment. This pharmacist will thoroughly review the history of present illness(s), past medical history, and comorbidities to help provider(s) determine the best course of medication therapy for the patient. This pharmacist will work very closely with Medication Reconciliation Technicians, inpatient pharmacists and providers to ensure provisions of accurate and safe medication treatments in a timely fashion throughout patients’ stay at hospital(s) to prepare for discharge. In addition to providing comprehensive pharmacy consult services to providers at every step in the transition of care process, the pharmacist will provide education and counseling to patients on proper medication use during the hospital stay and after discharge for a short period of time.
Review of patient medication lists to clarify and resolve discrepancies (omissions, duplications, dosing errors, drug-drug interactions, drug-disease interactions, etc) in a proactive manner with the patient’s primary care provider.
Participate on a pharmacy team focused on transitions of care services within the health-system including medication history collection, medication reconciliation during transfer points and patient and family health education.
Reconcile medication histories by reviewing information collected by other pharmacy team members.
Review drug therapy to ensure appropriate use, dose, dosage form, regimen, route, therapeutic duplication, drug allergies, and drug interactions.
Communicate with inpatient and outpatient care teams to ensure proper discharge medication plan and minimize duplication of effort.
Participate and/or lead in organizational performance-improvement efforts related to medication reconciliation including participation in the development of competency training and skills assessment for those performing medication reconciliation.
Serve as a resource to healthcare providers and participate on institutional committees as appropriate.
Provide accurate, safe, timely and appropriate medication therapy based on the patient age and needs.
Anticipate care transitions and medication changes in order to assist with transition planning regarding unusual, compounded or high cost medications.
Work with patient, family, caregivers and the health care team to identify and address barriers to medication adherence and access.
Assist with medication dispensing related issues, as needed or upon request for patients utilizing the bedside medication delivery service
Counsel patients or caregivers on medications regarding changes, side effects, and medication or food interactions, drug indication and dosing regimens
Collect and maintain data on metrics that will be used to evaluate the impact of pharmacy services on quality measures and patient outcomes.
Provide presentations, publications, and other informative activities in specialty area and other drug-related topics to the pharmacy and medical/health care community (e.g. professional organizations) and general public.
This pharmacist will additionally need to be skilled in the work of a clinical pharmacist to include:
Follows guidelines of the P&T committee for therapeutic interchange, and formulary management.
Monitors drug therapy for appropriateness and promote/ evaluate rational drug therapy that improves pharmaceutical care.
Must be competent in providing general pharmacy services that include anticoagulation management, renal dosing services and pain management interventions.
Will work with physicians, specialists and other clinicians to implement appropriate guidelines and protocols for drug use based on evidence based medicine, national guidelines, and literature based protocols.
May act as a preceptor to pharmacy students or pharmacy residents.
Participates in Performance Improvement (PI), Drug Use Evaluations (DUE), Adverse Drug Reactions (ADR) and clinical programs of the pharmacy to meet departmental and hospital goals.
Recommends changes to drug therapy based on patient assessment.
Provides and documents clinical interventions, therapy recommendations and other clinical activities according to department policy and procedure in order to maintain accurate and complete patient records.
Must maintain competency in pharmacy services that are delivered on general medical, surgical, geriatric and pediatric floors.
Provide prospective medication order review for all inpatient medication orders. In particular, verify all high risk medications to ensure the appropriateness of use from clinical, financial, and policy compliance perspectives.
