You will be responsible for performing drug regimen reviews and identifying medication related issues in both skilled nursing and assisted living facilities. Other services provided by the consultant pharmacist may include but not limited to attending psychotropic and monthly or quarterly quality assurance (QA) meetings, providing medication-related in-services, and auditing medication storage rooms. In addition to long term care consulting, on-site support and consulting at an acute psychiatric facility is required.
Additional responsibilities of the role include:
•Knowledge of all Long-term Care regulations and laws required by CMS and the Pennsylvania Department of Health. •Review all medication therapies of residents in skilled nursing facilities every 30 days and in assisted living facilities based on contract agreement. It will be the expectation of CPS and the skilled nursing facilities that the consultant pharmacist provides all monthly pharmacy recommendations in the assigned format. •Perform drug regimen review for all skilled residents based on the Federal F-329 (unnecessary drug guidelines) the current CMS State Operations Manual Appendix PP and general pharmacotherapeutic principles. Special emphasis will be on the proper indication, proper monitoring of therapeutic outcome and side effects, and identifying appropriate opportunity for gradual dose reduction of certain classes of drugs (e.g. psychotropics) •Quality Assurance reports will be generated and provided in the assigned format and presented at QA meetings as contracted with the facility. •Attend psychotherapeutic and infection control meetings as requested by facilities.•Provide on-site clinical support for a psychiatric facility. •Develop and present financial and medication-related in-services. •Maintain pharmacological knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. •Participation in the weekend and holiday staff pharmacist rotation located in Warrington, PA is required. The dispensing pharmacist’s responsibilities include, but are not limited to, preparing medications by reviewing and interpreting physician orders; detecting therapeutic incompatibilities; ensure accuracy of dispensed orders •Develop a good working relationship with pharmacists in other areas of the company to foster a team approach in assuring that operations are running successfully. A consultant pharmacist will also provide support to the Department of Clinical Pharmacy. Proficiency in the full Microsoft Office package is required to complete clinical assignments that may include development of in-service presentations, clinical writing, and analysis of pharmacy utilization. Public speaking and presentation skills are required.
You must have the ability to travel within 50 miles of Warrington, PA to perform onsite drug regimen reviews and attend QA meetings within the required timeframes.
Additional requirements of the role include:
•Bachelor’s Degree in Pharmacy or PharmD is required •Must have a current Pennsylvania state Pharmacy License in good standing •Excellent verbal and written communication skills •Ability to effectively speak and present in front of a group•Proficiency in Microsoft Office •Strong customer service abilities •Reliable transportation for travel within 50 miles of Warrington, PA. Mileage is reimbursed.
* Certified Geriatric Pharmacist (CGP) preferred
* 2+ years’ experience working as a Consultant Pharmacist preferred
